To register for 7Summits Camp, fill out the Registration Form (available by clicking on the Registration Form link on the bottom left-hand corner of this page) and send in a 20% deposit (checks made payable to Summit Vision) to:

Please note that our mailing address is different than our camp address!

The registration deadline for all camps is June 2, 2008. Full payment and all camper forms are due by this date. Children who are paid in full by March 31, 2008 will receive a $15.00 discount.

All campers will need to have the following completed forms on file before they attend 7Summits Camp:

Camper Health Form
Camper Profile
Camper Expectations
Transportation & Emergency Procedures
Medication Form (if necessary)
Camper Drop-off & Pick-up Procedures

All forms (except Camper Drop-off & Pick-up Procedures) are available by clicking on the blue icons to your left. You may send these forms in along with your registration or send them at a later date.

All forms are due by June 2, 2008.
